what do you guys think?

Those appear to be cecals. They should be reingested as they are a vital part of keeping a healthy GI. Most of the time, a diet too rich in protein will cause excess cecals. Overweight rabbits are sometimes unable to reach their rear end to eat them. Rabbits with mobility problems such as spinal injuries or arthritis can have problems. In rare cases, illness or infections can cause this. But the most likely is diet. I would review what he is being fed. The most common issue is too many pellets. A healthy adult should be eating grass hays free choice with limited pellets. In our rescue, we feed pellets only to maintain weight and body conditioning and we have rabbits that don't eat pellets. They get only high quality hay, fresh water and limited veggies. Make sure you are feeding a high fiber, low protein diet.....lots of hay, limited pellets and no treats.
