I just wonder why he is bald in a spot on his head.
I'm worried if he scratches, he will injure himself. is there anything I should do?

Is the hair loss in the same place he got the cat scratch, just to be totally clear?

You can fashion a collar to keep him from scratching but it will interfere with his ability to eat cecotropes and make him very upset.

Can you give us a picture of the spot so we can see if it looks like an abscess.
