Have you tried hand feeding him the other food? If he took a treat from your hands it might just be he's feeling a bit too shaken up to take food from the ground, not sure. Him being deprived of food for 12 hours before the surgery worries me a bit - do you have green veg to offer him? Carrots...

Toby was not neutered until about 1.5 yrs and he was fine, if that is any assurance :) Seconding what is said above - for males it is not a very major surgery; as long as he is healthy he should be fine.
